In connection with my voluntary involvement in activities undertaken for, and with the participation and support of Volunteer Center of Southern Nevada (an initiative of United Way of Southern Nevada), a non-profit charitable organization, I hereby agree, for myself, my heirs, assigns, executors, and administrators to release and discharge Volunteer Center of Southern Nevada and United Way of Southern Nevada, its officers and/or property as a result of my involvement in such activities, whether or not resulting from negligence, and I agree to release and hold Volunteer Center of Southern Nevada and United Way of Southern Nevada, and all social service agencies, schools, and parks, and their officers and directors, employees, agents, sponsors, donors and volunteers harmless from any cause or action, claim, or suit arising therewith. I hereby attest that my attendance and involvement in such activities in voluntary, that I am participating at my own risk, and that I have read the foregoing terms and conditions of this release. Furthermore, I grant permission for photographs, video and quotations from me during my involvement with Volunteer Center of Southern Nevada and United Way of Southern Nevada to be used to further promote volunteerism.

Additionally I agree to receive the Volunteer Center e-newsletter and other electronic communications regarding Volunteer Center activities and understand I am able to opt out of e-newsletter communications at any time.  Opting out of e-newsletter communications will still enable me to volunteer as I will continue to receive electronic communications regarding projects I sign up for.
